Referring Doctor Forms

If you are a PCP and would like to refer one of your patients to see us, please fill out the form below and fax it to us at 252-222-5705 along with all recent records pertaining to the reason for referral. We will schedule the patient for our first available appointment and inform you of that date and time.

Please keep in mind that we are no longer accepting new Medicaid patients.  

We are now accepting new Medicare and Tricare patients.

Bone Density Referrals

In case you have not heard, we are no longer accepting bone density referrals on a walk-in basis. If you would like to schedule your patient for this procedure please give our office a call to set that up. You will find the form you need to fill out attached below as well.
